I’m sorry that your node is disqualified. Please, try to figure out why is it happened to do not repeat the same mistake in the future.
You can use this guide to find out what was the reason:
Unfortunately unlike suspension the DQ is permanent and not reversible or recoverable.
So, to have a node again you need to start from scratch - with a new generated identity, new authorization token and clean storage.
And do not allow it to fail because of the same reason.
